Monthly Fitness Class Pass
WELCOME to your 1st month unlimited fitness classes!
- A one-time fee ($20) applies upon registration. May be paid at first class, billed separately, or for convenience, please select from drop-down menu.
- A partial-month fee may apply when Monthly Pass is initiated mid-billing cycle, (billed separately if applicable).
- Subsequent monthly invoices will arrive by email, due the 1st of each month.
- Applies to in-studio, outdoor, real-time virtual and/or hybrid class formats on Fitness Class Schedule, subject to change seasonally. No classes on Holidays.
- There is no contract. Provide 30 days notice of cancelation.
- Prior to my first class, please complete the Registration and Par-Q (health) forms (to be updated yearly), and sign the Fitness & Monthly Pass Policies.

With payment for this item "Fitness - Monthly Pass," I agree to Activate a Monthly Pass membership and understand the following.....
1) In consideration of my Monthly Pass membership at BusyLizzy Fitness, I agree to be billed in advance for each month of classes.
2) I have read the MONTHLY PASS POLICIES document and understand the policies stated including fees, due dates, cancellation and pause procedures, applicable classes and exclusions.
